Preregistration
We encourage all patients to preregister for scheduled services to ensure a quicker check-in process.
If you don't submit pre-registration online, you will receive a phone call from Allina Health prior to your appointment. If you are unable to answer that call, the representative will leave a message with a phone number to call. Please call back when convenient to complete your pre-registration.
To preregister online, simply fill out the form below and make sure all of the required fields noted with an asterisk (*) are completed before submitting your registration.
